1. Pursuant to Section 4 of Republic Act No. 3019 known as Anti-Graft and Corruption Practices Act and Rule III (Conflict of Interest) of the BI customized Code of Conduct, all accredited private entities consisting of travel agencies, law firms, private firms, missionary groups and their representatives, among others are hereby required to submit their respective (individual) duly accomplished personal data sheet (PDS).

2. The PDS which is prescribed by the Bureau of Immigration is available at the BI Accreditation Office located at the 2nd Floor, BI main building, Magallanes Drive, Intramuros, Manila.

3. Deadline for submission of PDS will be on September 1, 2011.

4. The Accreditation Office shall revoke the accreditation I.D. of those who fail to comply with this directive.

5. Let copies of this Circular be posted at conspicuous places within the Bureau premises, copy furnished the Philippine Travel Agencies Association (PTAA), BI Accredited Representatives Association (BIARA) and the office of the Resident Ombudsman.
